The Phone Reminder app in your Apple Watch will notify you when it looses the Bluetooth connection to your iPhone.
Just launch the iPhone app, set the Apple Watch App to run in the background and you are good to go.
Install the app in your iPhone and Apple Watch and make sure to:
1. Allow Notifications
2. Allow Bluetooth
3. Allow Location ALWAYS
