and complete the MDS in a collaborative environment with other clinicians in the facility to provide accurate PDPM calculations and classification. Note: Any MDS information created in the ACPlus MDS application cannot be transmitted to the CMS QIES database without 3rd party approval. To obtain approval and this functionality, please go to the QTSO website (https://qtso.cms.gov/access-forms/mds-corporate-and-third-party-access), fill out and sign the “MDS Third-Party Service Bureau User Request” form and email to ACPlusAdmin@hanger.com. Once approved by QTSO, we will turn on this functionality."
