Acuity Referrals

Acuity Referrals can be used to more easily send patient referral requests to our Acuity Eye Group team. You can quickly enter your patient's information and contact information and optionally the reason for the referral and preferred location/doctor. An Acuity scheduler will reach out to them same day to schedule their appointment.

With this app, you can be confident your patient gets prompt service for their eye care needs and that they will get scheduled for appointments with our Acuity providers immediately.

We also include educational information tailored towards primary care and general physicians. This includes general information about common eye diseases such as diabetic retinopathy, cataracts and glaucoma, as well as a cheat sheet to help translate our ophthalmic abbreviations (which often leave referring doctors puzzled when trying to translate the Ophthalmologist's chart notes).
