Through the Affinity Health Plan provider mobile app, you have real-time and easy access to your patients’ health care information and to services needed to prepare, plan, and engage in your patients’ care.

After creating your account, simply sign into the mobile app with your username and password. Once logged in, you can manage your healthcare insurance management services through your account, including:


• Researching the status of claims and authorizations

• Examining patient information

• Accessing your group’s account information
