目前虽然对糖尿病的病因认识还不清楚,但现代医学研究发展,在糖尿病发生和发展过程中有两个因素最重要的因素:胰岛素抵抗升高和胰岛功能下降。长期胰岛素抵抗,胰岛功能就会不断下降,胰岛功能下降到一定程度就会发生糖尿病。
在糖尿病发展早期阶段,胰岛通过代偿性分泌高水平的胰岛素,可以把血糖控制在正常水平,这时血糖、糖化、糖耐量等各项指标都正常,但胰岛素抵抗水平却不断升高,胰岛功能逐渐下降,直到胰岛代偿功能下降,不能分泌更多的胰岛素时,血糖水平才会慢慢升高。这个过程要持续10-20年,如果能提前发现胰岛素抵抗水平升高,就可以延缓甚至避免糖尿病的发生。
很多糖尿病患者只是在盲目服用药物,患者并不清楚自己的病情是否得到了有效控制,自己的病情发展程度如何。如果糖尿病患者的胰岛素抵抗水平不能及时降下来,胰岛功能就会继续下降,说明病情没有得到有效控制,需要及时调整治疗方案。
糖尿病高危人群,尤其是有糖尿病家族史的人,究竟会不会得糖尿病,关键在于胰岛素抵抗,如果胰岛素抵抗水平不断升高,糖尿病风险就越来越大。
胰岛素抵抗在多囊卵巢患者中也比较常见,但胰岛素抵抗的问题往往被忽略。
Calculate insulin resistance index and pancreatic β cell function index with fasting blood glucose and fasting insulin. Calculate average blood glucose with glycosylated hemoglobin. Calculate BMI with height and weight.
The increase of insulin resistance and the decrease of pancreatic β cell function are critical factors to the arise and development of diabetes. The increase of insulin resistance is also very common in polycystic overy syndrome.
In the early stage of the development of diabetes the fasting blood glucose , the glycosylated hemoglobin and the glucose tolerance maybe in the normal range but the insulin resistance level will increase gradually.
Lowering the insulin resistance level in time will delay and prevent the arise and development of diabetes.
