The Remote Patient Monitor app is designed to connect patients with their healthcare providers. The patient is able to communicate important health indicators to their provider via a secure cloud service. The cloud service analyzes uploaded indicators and allows healthcare providers to view reports and manage individual patients' treatment plans.
Currently, the app and cloud service work together to help patients and their healthcare providers treat hypertension (high blood pressure).
Hypertension is a global health problem and is a risk factor for stroke, heart attack, heart failure, dementia and kidney disease.
Hypertension Management
Using a home blood pressure monitor, patients take regular blood pressure readings. Readings are recorded using the app and transmitted to the cloud service for tracking and analysis. The cloud service tracks average blood pressure over different time periods and determines whether target blood pressure has been achieved. The app displays progress towards the lower blood pressure goal to the patient. Using a web portal, healthcare providers can review patient progress against their individual treatment plan. The cloud service automatically informs providers of progress towards achieving target blood pressure, and provides recommendations to review and adjust the current treatment plan if the goal is not being met.
Since the app is designed to work with the cloud service and provide healthcare providers with a means to manage patients' hypertension, your healthcare provider must register for access to the cloud service. Ask your provider whether they have registered for access to the service. If they do not have an account, they can check our website for information about how to gain access and invite patients to collaborate on their hypertension treatment.
