一、皈依：
皈依佛 皈依法 皈依僧 皈依十方三世常住三宝 （三遍）
二、礼敬：
那摩药师琉璃光如来（三遍）
那摩救胎地藏菩萨摩诃萨（七——百八遍）
那摩送子观世音菩萨摩诃萨（七——百八遍）
那摩一切救胎护生神圣众（一遍）
三、忏悔：
从前所作伤胎嫉生损命罪 皆由我执贪瞋分别愚痴爱
果报现前得遇善知方醒悟 今对佛前至诚惭愧来忏悔
四、供养：
徧地香涂鲜妙杂花敷 须弥四洲日月顶庄严
以此所缘诸佛佛土献 愿诸众生清净住胎藏
五、回向：
祈愿功德殊胜行 无边胜福悉回向
上报四重恩 下济三涂苦
但求一子来 安生护圣种
