What is it about?

HealtheCare remote patient monitoring cloud platform is designed for a variety of use cases including, but not limited to chronic disease care, post-acute care, aging at home, home hospitalization, self-check clinics, emergency care, rural health and more. The platform enables caregivers to enroll patients, enter their medical history and conditions, link them with one or more automatic care plans and utilize the platforms intuitive dashboards and data collection capabilities of the HealtheCare app to track wellness trends as well as the efficacy of their treatment plan. The app automatically manages and collects information from the patient including clinical questionnaires, symptoms, vital signs, medication compliance and enables bi-directional communication and video conferencing.
