The Instachart Mobile app gives residents and their families access to relevant medical
information (diagnosis, treatment, medication details, care plans, discharge plans, etc.).
Residents and family members can communicate securely with their physicians and caregivers
with any questions or seek advice through the app. The platform also supports the resident and
their family members post-discharge with access to discharge plans, continued communication,
and easily sharing of records with their community physicians.
