iUGO Care

What is it about?

The iUGO Care app was designed to support patients who are managing chronic conditions like Congestive Heart Failure, COPD and Diabetes at home. The app provides patients with reminders to perform critical tasks like collecting vital signs, taking prescribed medications, engaging in fitness or rehab activities and more. The app works with a range of sensors and wearable devices to automatically upload data collected in the home to the secure iUGO Care cloud, where it can be shared with family and members of the patient's clinical care team. The iUGO Care app also delivers patient education material and provides a calendar that tracks upcoming medical appointments and home visits. The app allows patients to securely send messages to members of their care team and supports secure videoconferencing or "eVisits" between patients and their care providers. iUGO Care empowers patients, allowing them to better manage their chronic conditions at home to improve health outcomes and reduce healthcare costs.
