The Lincolnshire VitruCare Platform is an online service available on smart devices to help put you in control of your health and wellbeing in a new and exciting way.
The Lincolnshire Digital Platform is for people living in Lincolnshire. The platform is managed by the Lincolnshire Sustainability and Transformation Partnership (STP).

Transforming the way care is delivered, the Lincolnshire Digital Platform is a digital support tool available to people in Lincolnshire who require support for their mental health issues as well as to individuals who have, or are at risk of developing a long term condition and for all citizens who are accessing social prescribing. The care provider will be able to ‘prescribe’ appropriate digital services to meet individual needs, delivering a ‘service for life’ that evolves with changing healthcare needs. Care teams supporting individuals; including those from the NHS and the third sector, will be able to interact with patients through the same digital service, allowing them to operate new care pathways and manage their patients in a more productive and highly patient focused way.
