The classification of pulmonary lesions in terms of malignancy is at times controversial. A system to predict malignancy of these lesions would be helpful in making decisions for subsequent surveillance and therapy. We developed the ‘LIONS PREY’ (Lung lesION Score PREdicts malignancY), a new and simple logistic scoring system, which precisely predicts malignancy of pulmonary nodules in percent. All patients that were presented with a pulmonary lesion to the MDT of the Department of Cardiothoracic Surgery at the University of Cologne/Germany, were prospectively evaluated. Availability of pathological result was mandatory upon study inclusion. After uni- and multivariate analysis eight relevant score parameters were identified. MDT members could use the new score to reinforce and facilitate decision making. Especially inexperienced physicians have a tool that helps them focusing on essential parameters. Patients may find it easier to consent to surgery knowing the likelihood of pulmonary malignancy. We presented the ‘LIONS PREY’ at the European Association for Cardio-Thoracic Surgery (EACTS) Annual Meeting held in Barcelona in October 2021.
