myCarepod

myCarepod is a unique healthcare app which provides comprehensive, configurable and secure tools for patients and their healthcare providers to engage in telehealth-driven coordinated care. The app connects to providers’ systems and can be configured to include a variety of features for self-management, a full or partial health record, specialised data capture, care plans and educational material for different long-term and acute conditions. Data is always held securely in your healthcare providers’ system.

The app itself is free to download, but you will need a registration code to be able to use the app. This will have been sent to you by your healthcare professional.

myCarepod enables you to:
• Access and update your personal health history including personal details and special needs, contacts, allergies, problems, procedures, medications, immunizations, lab tests, appointments, and multi-media content – including images, audio, video, documents
• Engage with personalized care plans: keep track of your regular activities and get timely reminders of actions you need to perform
• Maintain health diaries for specific conditions, such as Diabetes
• Create photo & video diaries to share with your care team
• Connect to your wearable devices including Fitbit, Withings and Garmin, and see data trends from integrated health trackers
• Get feedback on your data and trends based on rules set by your healthcare professionals
• Receive and complete assessments at appropriate points in your care
• Get relevant educational material as documents, videos and URLs, when you need it
• Securely communicate with and enlist the support of your Care Circle – family, friends and your health care team.
• Book appointments with your healthcare providers
