It only requires a connection to the internet (Wi-Fi or cellular data).

What personal information does the app require?
The application does not require any personal information from the user. All users have an existing account with us and must log in to use the application.

Beyond the permissions already described, what other disclosures and user consent steps does the app contain?
The application does not prompt for any consents as this application is an add-on to an existing subscription based electronic health record system where the user has already agreed to the terms and conditions of use.

Why do users access confidential patients’ medical records?
The Healthcare professionals using this app require patient information from PHIX to view the patient’s past immunizations, allergies, and any history of past adverse reactions to immunizations to ensure the patient’s well being. All patient information shown in the application is read only and is not stored on the mobile device. Patient information can only be entered or updated by using the full Web/WPF versions of PHIX.
