What is it about?

The app helps patients accomplish four evidence-based patient safety “to-dos that are” recommended by experts associated with the Minnesota Alliance for Patient Safety (MAPS): First, the app allows patients to keep a medication list that they can share with their health care team. Second, it allows patients to keep and monitor a list of warning signs associated with their condition(s). Third, the app helps patients gather medical test results so they can share those results with their health care team. Finally, the app helps patients remember to promptly follow-up on their health care team’s recommendations. The app allows patients to accomplish these patient safety to-dos via text input, voice recognition software and smart phone photos.
