The App was developed by the Palliative Care Team of Airedale NHS Foundation Trust to help clinical staff in the delivery of the 5 Priorities of Care.
The 5 Priorities of Care, were developed by the Leadership Alliance for Care of Dying People, to help focus on the needs and wishes of the dying person and those closes to them.
