PRISM is America's first virtual care program dedicated exclusively to stroke survivors. It was developed by stroke experts at Mount Sinai hospital in New York City, one of the world’s leading stroke centers, and is now being offered to patients and those who care for them across America.
The PRISM App allows users to seamlessly communicate and share health information with their PRISM care partner, a dedicated healthcare professional who is assigned to them as part of the PRISM program to guide them on their recovery journey.
Patients who have signed up to the PRISM program through their hospital or via the PRISM website (myprism.ai) will be provided with a password to access the app. Once downloaded and active, the PRISM app allows users to:
- Complete enrollment steps
- Create a patient profile
- Capture and share blood pressure and heart rate data
- Complete daily to-do lists
- Gain access to useful stroke education
- Track important stroke recovery information over time
- Schedule appointments with care partners
- Have video meetings with care partners
