Key Features:

- Easily pair application with your Bluetooth or Cloud devices for vital reading uploads or use your phone camera to take a picture of your readings.

- Read or view your vitals in a daily, weekly, and monthly format.

- Know what vital readings are needed for each day and receive reminders when readings need to be taken.

Program enrollment through Humana is required to use this application. Only you and your authorized care team will have access to your private data, which will be kept safe and secure.
