SageSurfer is a confidential platform that engages consumers to have a stronger voice in their care, enhances collaboration among members of a person's professional and personal care team, and offers aggregate data in real-time to identify needs and trends. Information is seamlessly integrated with your electronic health record and is HIPAA Compliant. The app also integrates with HealthKit to obtain health and fitness data after obtaining appropriate consent from the user. This allows us to build longitudinal records about the user. We collect users' physical activity, sleep, heart rate, and blood pressure data through Healthkit. Multiple pieces of research have found that this data is correlated with one's mental health.
