The Silverscore app, from researchers at Yale School of Medicine and NYU School of Medicine, calculates predicted risk of hospital readmisson and mortality (for patients ≥75 years of age) within 6 months of hospitalization for acute myocardial infarction. The calculator was developed based on findings from the NIH-funded SILVER-AMI study.
Silverscore.org incorporates traditional risk factors and several novel factors (functional mobility, hearing impairment, patient-reported health status) relevant to older adults, in order to provide patient-specific risk estimates that can inform prognosis and post-discharge management.
