The Patient Reported Measures is an application to be completed by the patient. The App collects data (using standardized patient reported measures) and offers an “editable” service document for the professional to finalize. The narrative information provided by the app may be inserted into EMR or final documentation. The report also includes a section for the physician’s assessment of laryngeal impairment.

How does it work? The App uses visual analog scales and choice boxes provided in the categories of Patient Data, Voice-Related Quality of Life, Glottal Function Index, Vocal Health, and Reflux Symptom Index. Ratings are calculated and reported in a brief service document. The document can be edited and placed in EMR or paper centered report.

Benefits. Use of this app will improve consistency of patient rating of voice limitations and provide time savings in report writing. Students will benefit from the observation structure and guidance provided for the writing of a final clinical report.
