Through the Affinity Health Plan member mobile app, you have real-time and easy access to your latest health care information and to services needed to plan, understand, and participate in your care.

After creating your account, simply sign into the mobile app with your username and password. Once logged in, you can manage your care through your account including:


• Researching the status of claims and authorizations

• Making your premium payments

• Accessing your medical information
