This app is intended for patients registered at a clinical service which has invited them to download the app to manage their condition remotely, complete assessments, and access support resources to improve their health and wellbeing.

The app may also be used for research studies who register participants onto the app to take part in projects.

A member of staff will provide users with their login details or instructions on how to sign up for the app. Please contact a member of staff at your local health service or research project for support if you get stuck.

C19-YRSm comes with an optional subscription service where you can self-refer yourself to access the app. You may purchase an auto-renewing subscription through an In-App Purchase.

• Your subscription will be charged to your iTunes account at confirmation of purchase and will automatically renew (at the duration selected) unless auto-renew is turned off at least 24 hours before the end of the current period.
