With Avenue 360 Virtual Visit, you receive quality health care from the comfort of your home. Our professional, board-certified doctors, licensed nurse practitioners, and licensed professional counselors are here via your computer or phone to provide quality, compassionate care.

To get started, download the Avenue 360 Virtual Visit app, set-up your account and you’re ready to go. Our providers are available during regular business hours (Monday-Friday, 8 a.m.–5 p.m.).

During your virtual visit, our providers will offer diagnosis, and if necessary, prescribe medication. Virtual visits are available for primary care, adult medicine, pediatrics, mental health, and women’s health. Some conditions may require in-person examination.

If you are an Apple Health user you will have the option to share important health information with your provider during your video visit. Sharing Apple Health data is optional but allows the provider to see heart rate, weight, and other health information to help them make a more accurate diagnosis.
