Heart Failure Facts (according to the Heart Failure Society of America):
- About 6.5 million adults over the age of 20 in the US have heart failure
- There are nearly 960,000 new cases of heart failure every year
- The number 1 cause of hospitalizations in the Medicare population
- Approximately one-half of heart failure patients die within 5 years of diagnosis

CardioVitals is designed to allow patients to work more closely with their healthcare providers in the fight against heart failure. Along with medications, lifestyle changes and following medical recommendations, the CardioVitals app can help with managing excess water buildup (measured as additional weight).


What do we provide?
- Using your iPhone, you can measure cardiac vital signs or “cardio vitals” (ie weight)
- Set reminders for when you need to take measurements
- Assess your progress over time
- Your data travels with you if you need to change healthcare providers

What we are not?
- We do not provide independent recommendations regarding your recorded measurements. The purpose of this app is to allow better communication between patients and their established healthcare providers so they can optimize their heart health with the advice of their healthcare provider

Who will benefit from this app?
- Anyone with congestive heart failure
- Patients with heart failure or excess fluid build up

How do you begin the journey to better heart health?
- After purchasing the app in the app store, you will sign up with your email address and set a password to protect your information
- Modify your settings to personalize how and when you want to record your information

Future Additions:
- Analysis and optimization of heart health with the assistance of your healthcare provider
- Tracking blood pressure
- Tracking heart rate
- Share it with your healthcare provider or multiple providers

Subscription Info (in a future release):
- Payment will be charged to iTunes account at the confirmation of purchase
- Subscription automatically renews annually unless auto-renew is turned off at least 24 hours before the end of the current period
- Account will be charged for renewal within 24 hours prior to the end of the current period, and identify the cost of the renewal
- Subscriptions may be managed by the user and auto-renewal may be turned off by going to the user’s Account Settings after purchase
