Care Management Provider

The Care Management Provider app connects physicians to their patients outside of the boundaries of a medical facility. This online platform helps the physician to view his / her patient's records and data via a care management portal. The app helps the physician to be updated about his / her patient's medical progress no matter the geographical location of the patient. The physician will be able to view the following details of the patient with just a single tap on the screen:

- Patient Demography

- Medical history

- Current medications

- Care plan

- Progress notes

- Upcoming appointments

The app also notifies the physician of any changes or updates on the patient's profile. The app allows the physician to send in consultation requests, make referrals and schedule appointments for the patients with the assigned care coordinators.
