ChronicDoc

ChronicDoc™ was created as a solution to the administration for patients who suffer from chronic medical conditions. Conceptualized by a general practitioner and taking in to account practical experiences, we aim to provide convenience to both doctor and patient and make the follow up process more efficient. ChronicDoc™ is based on an existing relationship between doctor and patient and automates the entire follow-up process from reminder to script signature and delivery.

Our main features include:
* Separate profiles for doctor and patient, linked at registration with unique features per user.
* Follow-up reminders for patients are set in advance by their treating doctor.
* Patients are able to make use of the built-in health journal to document simple health data such as blood pressure and blood glucose. A weekly mental health check-in and pain journal is also included. This helps the doctor decide how well controlled a patient is.

* We have designed a set of follow-up assessments per chronic condition to guide the doctor on how well controlled their patient is.
* Patients are able to elect a preferred pharmacy on registration.
* Paperless scripting for doctors using advanced electronic signatures requiring 2 factor authentication (in-person onboarding required upon registration).
* Scripts are sent automatically to a patients pharmacy and the patient is notified for collection or delivery.
* Once one follow up interval is completed, a doctor is able to preselect the next follow up interval for their patient ahead of time.
