For patients of healthcare providers in the Connect4Life network. This app enables communication between you and your doctor. It allows your doctor to provide the specific information that is related to your health and health concerns.

With this app you can:

•Make requests to your physician (e.g., refills, appointments, test results).

•Receive personalized messages from your physician. (e.g., appointment reminders, alerts as your health status changes).

•Receive announcements (e.g., vaccinations, workshops, health alerts).

•Receive personalized medical education for your health. This includes information on diagnosis and treatment.

•Track progress of existing conditions with data sent to your physician and systematically monitored to alert your doctor.

•All of this information is automatically transferred to your physician electronically via a secure HIPAA compliant web portal.
