COVID Wellness Nurse

Using this application, COVID-19 patients or individuals who have come in close contact with someone with COVID-19 can update their healthcare provider on their health status daily by responding to a series of questions related to the presence or absence of symptoms.

The purpose of the application is to serve as a monitoring and engagement program for healthcare providers, allowing them to manage the health of their patients over time by using the information collected to analyze their patient’s current health status and to send messages or alerts to them based on the care they need.

Furthermore, the data collected by the application are later aggregated by your healthcare provider for the purpose of discovering trends at the population level potentially requiring future action or intervention.

This application is limited to individuals whose healthcare providers are enrolled in the Digital Wellness Nurse monitoring program.
