Doc4Me is a mobile application developed by The NASPGHAN Foundation in collaboration with the Crohn’s and Colitis Foundation to help adolescents and young adults with inflammatory bowel disease (IBD) identify an adult practitioner to whom to transition from pediatric to adult-centered care.
Doc4Me, will assist patients in finding a physician who treats adult IBD patients.
It will also help prepare them for their first appointment with their new doctor and provide them with links to available resources to improve their transition process.
The database of GI practitioners who are listed in the app represent all 50 US states.
Doc4me integrates with Apple's Health- and Carekit systems so you can keep track of your Careteam in the app.
