Patients at participating practices can use eConsult to keep their NHS GP updated about their long term condition, such as high blood pressure.

As a registered patient, you can use eConsult to sync your health data from your phone or monitoring device via your iPhone's Health app and request feedback about your condition from your own GP. Regularly sending your health information to your GP may mean fewer face-to-face appointments.
You can also manage your condition by viewing approved self-help guides and videos.
