As simple as it sounds. This app simply allows you to take a photo (with a date and time stamp), write a note associated with the photo. After you're done taking photos you can simply create the PDF using your device and either send it as an email or store it in the cloud (including iCloud, Dropbox, or any other cloud services).

Additionally, you can edit or add headers and footers to your document also from within the app.

Everything is laid out in a User-Friendly manner. However if you're lost in any page of the app there's a small white info button at the top right to help guide you.

The created PDF could be editable later on since the created objects are not flattened. Using the right tools the PDF file could then also be extracted to a Word document to be edited in an easier manner.
