Companies and organizations register individuals in the HC Disease Surveillance system, allowing the individuals the ability to remotely self-report infectious disease symptoms, as defined by the CDC. Individuals can complete infectious disease screenings by self-reporting symptom checks and temperature prior to entering work or a public space. Individuals with symptomatic responses are immediately told to stay home based on their symptoms and are flagged in the system for review and follow-up by administrative users. When an individual has a symptomatic response, the employer or organization can monitor symptoms by having the individual continue reporting daily symptom checks during a designated isolation period. This helps companies and organizations determine satisfaction of CDC guidelines to return to work or entrance into a public space.

The HC Disease Surveillance system can also be used in cases where an individual has possible exposure to an infectious disease. By allowing the individual to report daily symptoms, companies and organizations can monitor symptoms during a defined incubation period to help define next steps including additional monitoring of other possibly exposed individuals.
The Disease Surveillance notification system sends out daily reporting reminders for the designated incubation period, and allows the PUIs to remotely report their information from a smart phone or any web enabled device. Case managers save time with the ability to manage by exception cases that either fail to report or become symptomatic, needing further attention and referral to a medical facility.
