App Store Description

The Heart Failure Monitoring app allows geriatric patients with heart failure to record their weight
and to describe their physical condition. The premise of the app is to help patients to be able to better monitor their weight and signs of HF. The application will be used to test a group of patients (roughly 60-120 patients) with HF who are cognitively stable. Patients will be provided with an iPad Mini and asked to enter their weight. When a patient begins a weight survey, they’ll be asked a series of other questions to better track their daily condition. When done with the survey, the patient will be able to review their weight over time on a dashboard display. The HF is developed on multiple platforms including iOS, Android OS, and Windows OS.
