Patients often travel to the Inherited Cardiac Conditions (ICC) clinic from distant locations, but require follow up tests and reviews at their location hospital and GP centers. These are typically arranged by paper-based communication which inevitably has challenges including factors such as time and reliability.
Hospital- Q is a mobile App to help patients to track and monitor these tests and medication reviews which they require when leaving the ICC clinic. This will provide a link between the patient, their planned tests and medication reviews and the ICC clinic team.
At the moment, the app is used by the team at the John Radcliffe Hospital - Oxford University Hospitals and their patients. If you are not a patient from the ICC at the John Radcliffe Hospital, you may still use the app, however please note that your data will not be monitored by the clinical team.
