MedForm

MedForm makes it easy to complete medical forms and questionnaires on your iPhone or iPad. It is designed to be used in the clinical setting or by patients outside the clinic where results are to be reviewed by your doctor / health professional.

MedForm will ask for the language you prefer to use in order to complete the survey. To protect your privacy MedForm will only ask for your date of birth and the initials of your first and last name. The answers will be sent to the doctor / health professional via a secure cloud service and moved into the doctors system. No data remains online.

Doctors and health professionals can send push notifications to MedForm to give you timely reminders such as when to complete the next survey or questionnaire.

Questionnaires can be uploaded to the MedForm system by practitioners who have a MedForm membership.

MedForm uses the power of Apple ResearchKit to present questions in a clean and consistent format and to ensure the data returned is accurate and timely.
