MediCCM allows clinicians to quickly track time spent with patients enrolled in the Medicare Chronic Care Management (CCM) program.

Use the mobile app to access your patient data from MediCCM and record the time and notes of the non-face to face (NF2F) care provided to the patient.

This application is great for quickly logging time after you've spoken to a patient on the phone, sent an email asking your patient about their progress on the care plan you helped them complete to better manage their health, or any other continuing care you performed that falls under the Medicare Chronic Care Management program guidelines.
