To use My Dose Coach™, your healthcare provider must be registered in the program and send you a dose plan.
My Dose Coach™ Basal Titration is designed for adult patients who have type 2 diabetes and who have been prescribed and are taking a once-daily long-acting basal insulin by their healthcare provider. You will then be able to us My Dose Coach™ as an aid to track your fasting blood glucose levels and adjust your long-acting basal insulin dose based on your healthcare provider's plan to help you work toward your target glucose level.
My Dose Coach™ Maintenance is designed for use by patients to enable logging of insulin, non-insulin medication use, and blood glucose measurements and provide dosing and measurement reminders. Essex Maintenance module may receive values transmitted by supported accessory wireless devices.You will then be able to us My Dose Coach™ Maintenance as an aid to track your blood glucose and diabetes medication manually in the app or with connected bluetooth devices.
Important Information
My Dose Coach™ Basal Titration is indicated for single patient use for adult patients with Type 2 diabetes who have been prescribed and are using a once-daily long acting (basal) insulin. My Dose Coach™ is a tool that calculates a dose change based on an individualized dose plan the healthcare provider has created. My Dose Coach™ is not intended to replace the healthcare professional's care or advice. Patients should talk with their healthcare team if they have questions about their treatment. My Dose Coach™ Maintenance is indicated for use outside of the clinic setting by patients 18 years of age or older who have been diagnosed with either Type 1, Latent Autoimmune Diabetes in Adults (LADA) or Type 2 diabetes and who are administering once-daily, long-acting basal insulin or who are currently using multiple daily injections (MDI), via syringe, pens or similar therapy. It may also include
diabetic patients with inadequately controlled diabetes on oral anti-diabetic treatment.
