My HealthCheck

The digital HealthCheck uses the same questions and measurements as the NHS Health Check, but can be done on the move or in the comfort of your own home.

HealthCheck is designed to spot early signs of stroke, kidney disease, heart disease and type 2 diabetes in adults aged 40-65. As we get older, we have a higher risk of developing these conditions, the HealthCheck helps you find ways to lower your risk.

HealthCheck uses a combination of evidence-based questionnaires, physical measurements, and blood tests to generate personalised health advice to help maintain and improve your health.

Once you have given consent, your personal health information is only ever shared with your GP, so they can provide additional support and treatment if necessary.

Features:
Personalised HealthTrack that maps your health behaviours (including time spent a work or home and walking or sleeping)
Integrates with the Health app to provide tailored advice on your activity levels (such as step count, walking speed and waking hours), and integrates with measurements you have already provided to pre-fill your questionnaires.
Digital activity, health and lifestyle questionnaires (Including AUDIT and GPPAQ)
Self-reported blood pressure readings and body measurements (including waist circumference, height and weight)
Blood tests requested from your phone (including Lipid profile and HbA1c).
Personalised dietary, medical, lifestyle and exercise advice (including links to Apps and local services to support you make these changes)

The digital HealthCheck has been developed by Eos Analytics in consultation with GPs and Public Health England.
