App Store Description

Dementia is a life changing condition mostly in later life and is increasingly recognised as an important clinical and social problem with devastating consequences for the patients and those who care for them. Accessing appropriate information and services for people with dementia and their carers is vital in their continuing care and management.

It is intended that the app can be used by people with dementia and their carers at any point or stage of illness so that they can use guidance to help them to manage various aspects of their life and more importantly signpost them to an array of services and organisations that provide specialised support and information, locally, regionally and nationally. There is also information for dementia risk prevention. Signposting information is categorised, such as, finances, transport, employment, living well, relationship etc. with contact details for these resources and links to websites such as Alzheimer’s Society, NHS choices and YoungDementia UK.

The Dementia app benefits patients and families, carers and health professionals too. The app aims to empower patients and carers with access to appropriate information, knowledge and support following diagnosis.
