Prior to downloading the application, please confirm that you have been enrolled as a participant in the Pathway2Health℠ program with your provider.



TeleHealth Suite is a leader in providing healthcare technology, including the Pathway2Health℠ product. The participant portal focuses on engaging and empowering participants to view biometrics received from integrated devices such as smart watches, view and complete individualized care-planning tasks and receive reminders for appointments and medications. The participant portal’s communication center provides access to your health care team and contacts via messaging and video visit.



Pathway2Health provides the ability to connect to Apple Health. With this, you can download the biometric readings from your device and the health data belongs to you from different health organizations to Pathway2Health.
