The Seasons Hospice Referral App makes it incredibly easy to collect all the information you need to refer your patient to our service. Our staff are standing by 24/7 to help support you and your patients every step of the way.

Our app is focused on providing you with the tools you need to support your patients as they transition to hospice, including:
• Dedicated referral forms for physicians, as well as a form specifically for community partners like facility administrators/activity directors/discharge planners. We understand that professionals in different areas may notice unique signs in their patients, and so our referral process is built to ‘speak your language’. You can even upload facesheets and clinical photos, right from the app!
• Email confirmations for any referral you make, as well as an in-app listing of all of the patients you have referred to our services.
• Prognostic guidelines for hospice eligibility, with easy-to-understand videos that demystify eligibility requirements for multiple disease types.
• Ability for you to request a visit or a call from a Seasons representative.
• Resources to help educate patients and their families on the benefits of hospice care
