Skye Health App (by Solara Health) is a patient-centric mobile app designed to educate, guide and support hospital patients, enabling them to become active participants in their healthcare journey. The platform empowers you to proactively manage your health by providing direct access to clinical expertise, self-management tools and support resources.

The Skye Health App enables you to:
• Access evidence-based articles & videos relating to your diagnosis
• Answer health questionnaires (e.g. quality of life)
• View tasks and reminders prescribed by the hospital (e.g. exercise, nutrition, medication)
• Manage your health data
• Contact your care team
