SoteriaMe

SoteriaMe is a smartphone app that brings patients and clinicians together through a secure messaging service and gives patients access to their personal health records. Their visit history, together with a list of their current medications, active problems, allergies and latest vitals across the continuum of care, is displayed on a digital dashboard. Patients can complete satisfaction questions, join support groups, monitor their adherence to care plans and gain access to wellness reports.

To support clinician interaction with patients through SoteriaMe, clinicians have access to the Soteria Clinician Portal. In the Soteria Clinician Portal, healthcare providers can initiate patient engagement and education, and monitor patient reported outcomes.

SoteriaMe and Soteria Clinician Portal leverage Soteria Mobile, a native iOS application available in the App Store, that supports optimal clinical documentation and natural clinical workflows. The advanced configuration capabilities of Soteria Mobile ensures that the SoteriaMe app is only made available to patients who are in the care of clinicians who use Soteria Moble. Levels of authentication have been built into the registration process so users can be assured that their health information is private and secure.

Patient information is synchronized across all Soteria platforms, preserving data integrity and reliability.

Registering for SoteriaMe is made possible through a Soteria Mobile healthcare provider.

Follow these simple steps:

1) Download SoteriaMe from the App Store.
2) Your healthcare provider will send you a registration code to activate your SoteriaMe profile.
3) When you open SoteriaMe for the first time, you will be presented with a registration form (Step 1 of 3 steps).
4) Complete all the fields in the form, including the Registration Code from the SMS sent to you by the clinician.


Note: Two factor authentication will be required through step 2 and 3.

Once your registration has been completed, you will be able to do this and more:

• Log in to SoteriaMe on your smartphone.
• Tap My Clinical Records to gain access to your visits, diagnoses, vitals, allergies.
• If your healthcare provider has configured this option, a series of wellness reports.
• Receive messages from your healthcare provider to support optimal care and information sharing.
• Message your healthcare provider questions or concerns.
• Be part of a support group if your healthcare provider has discussed and activated this for you.
