「Sprout2Sync行動應用程式」為「善思回診提醒關懷計畫」中的元件之一。其開發的主要目的係與「SPROUT照護管理系統」相互整合，協助醫師及護理人員針對思覺失調症病患進行整合式回診管理，進而減少因中斷用藥的復發風險。

「Sprout2Sync行動應用程式」主要供醫護人員照護病患使用。其主要功能如下：
• 掃描病患專屬的QR code貼紙來記錄思覺失調症病患的回診以及住院狀況
• 上傳受試者同意書至「SPROUT照護管理系統」
• 協助醫護人員在病患回診治療或住院時進行CGI-Severity及社會功能評估 (PSP)評估
