The Dog Allergy Survey

The Dog Allergy Survey Application is an application that collects data from users regarding dog allergies in themselves and their families.
The user is recommended to create an account using a functional email address (do NOT use school or work emails that may block external emails), and complete the Dog Allergy Survey in regards to themselves, their biological parents, and their biological grandparents.
There is an option to withdraw from the survey if the user later decides to do so, and all of the survey responses are completely anonymous.
