This application has been developed and funded by AstraZeneca (a private limited company registered in England and Wales under company number 03674842 and with registered offices at 1 Francis Crick Avenue, Cambridge Biomedical Campus, Cambridge, CB2 0AA) who is represented in the U.K. by AstraZeneca UK Ltd with offices at Horizon Place, 600 Capability Green, Luton, Bedfordshire, LU1 3LU. The service is supported by trusted third party vendors, who provide and support the technology. This application is for patients who have been enrolled to use it by their healthcare professional. The main purpose of the application is to help patients manage their prescribed medication in conjunction with their healthcare professional. The application will capture the prescribed dose and medication use data. Based on a patients specific medication use pattern (adherence data), they will receive regular medication reminders and missed medication messages. With prior consent, questionnaires may be forwarded to you through the application to further assist with understanding current management. The application will also help AstraZeneca and its partners to improve the services offered to patients and the NHS.
