Your Care Card

The Care Card was created for all the times that your loved one with Alzheimer's or dementia is receiving care, but you're not there. It is a one-page snapshot of their needs, preferences, likes, and dislikes - designed to help any care provider better understand them as a person and how to make them more comfortable. It keeps everyone on the same page.
