By doing so, we aim to support optimal cardiac and onco rehabilitation and enhance patients' quality of life following procedures such as surgery, stent placement, or during the management of chronic heart conditions like hypertension, or arrhythmias.

The MoveSmarter Health Application is a companion for patients managing chronic conditions at every step of their treatment journey. It keeps patients motivated and active with a personalized, doctor-validated exercise program. The app regularly tracks progress and vitals, sending real-time updates to the patient's doctors and care team for seamless and coordinated care.

Patients can now log key health metrics, such as water intake and sleep duration, while their physical activity levels are automatically tracked. This data is shared with doctors in real-time, enabling more personalized care. Additionally, patients can complete surveys sent by their doctors, offering deeper insights to support their ongoing health management.

To access MoveSmarter Health, download the app for free, then you will need an invitation code to the app, which includes a secure, personalized digit code sent to your email by your physician. The email will also contain the institutional code provided by your affiliated clinic, ensuring seamless registration and access.
